Medical Gas Re-Certification
Download the Record of Continuity of Brazer Qualification form (left). Please have this form copied onto your company's letterhead and signed by your Supervisor/Foreman (he must have witnessed you make a braze in the past 6 months.) The most recent date you put on your form will be your renewal date.

Effective 9/1/02, everyone must take the '02 Medical Gas Upgrade before they can re-certify. If you have not taken the '02 Medical Gas Upgrade, please call the school and we will enroll you in the next class.

If you have not made a braze within the past 6 months, please call and we will schedule a date and time for you to come to the school for this procedure.

NOTES:

  • The cost to recertify is $20.00, if continuity is current, payable to J.A.T.T.
  • If your card has already expired, the "DATE OF MOST RECENT BRAZEMENT" on your continuity form MUST be before the expiration date on your card. The cost, if expired is $30.00 for each continuity submitted.
  • If your Medical Gas Certification is not with the United Association, then we cannot re-certify your card. You will need to contact the company or school that certified you ORIGINALLY.


UA Weld Re-Certification
Download the UA Weld Continuity Form (left). Have your foreman complete the form and mail or fax it to this office. Make sure your date on this form does not exceed your expiration date on your UA Weld card. Continuity forms must be turned in every six months or less to prevent loss of your certifications.
